Job Description:
Upward Health, a pioneering provider of home-based medical care
for complex patients, is seeking a dedicated Supervisor for
Clinical Operations to oversee our dynamic clinical team. This role
is pivotal in ensuring the delivery of exceptional care to patients
primarily in their homes, enhancing their quality of life while
managing chronic conditions. The ideal candidate will bring a mix
of compassionate leadership, operational expertise, and a profound
commitment to transforming patient outcomes through innovative
healthcare solutions. Lead and supervise a multidisciplinary team
of healthcare professionals delivering home-based care. Ensure
compliance with healthcare regulations and standards while
maintaining the highest levels of patient care. Develop, implement,
and evaluate operational policies and procedures within the
clinical environment. Coordinate with healthcare professionals and
ancillary staff to manage patient treatment plans effectively.
Monitor and optimize staff performance through regular feedback,
trainings, and performance reviews. Manage patient care logistics
to streamline operations and enhance service delivery efficiency.
Facilitate communication and collaboration among team members and
external partners to achieve healthcare goals. Prepare reports and
analyze care delivery outcomes to drive improvements in clinical
operations. Strong leadership and team management skills Excellent
organizational and communication abilities Proficient in healthcare
management systems and patient care software Adept at conflict
resolution and problem-solving Knowledge of healthcare regulations
and accreditation standards Ability to multitask and manage
