VOCATIONAL NURSE DEPARTMENT: NURSING JOB SUMMARY: Assists the
professional nursing staff in providing maximum Patient care,
assessment, planning, implementation, and Evaluation of each
assigned patient. Under supervision gives And indirect patient care
as assigned. OSHA Blood Borne Pathogens Class I. NOTE: The current
opening is for a PRN employee (not part-time or full-time).
ESSENTIAL DUTIES AND RESPONSIBILITIES RATING A. Utilizing the
nursing process, completes a basic physical and psychosocial
assessment of patients and takes nursing histories to provide a
basis for development of patient care plans under the direct
supervision of the licensed professional. Identifies
teaching/learning/and discharge needs of the patient upon admission
and throughout their hospitalization. Provides a basic assessment
which accurately reflects overt physiological and psychosocial
needs of patients and reports these findings to the appropriate
personnel, (RN). Uses input from patient, family, significant
others, physician, and other health care team members. Provides
care appropriate for the pediatric, adolescent, and geriatric
patients served based on demonstrated knowledge of the principles
of growth and development over the life span. Assesses and
interprets data on pediatric, adolescent, and geriatric patients’
status to identify patients’ requirements relative to their
age-specific needs and provides care as needed and as stipulated by
departmental policy. B. Using the nursing process, plans and
provides patient and family care and evaluates patients’ response
to the care provided and redirects care as necessary under the
supervision of a Registered Nurse. Assists with formulating
appropriate nursing diagnosis and identifies patient problems from
admission data base and ongoing assessment. Establishes patient
outcome goals that are realistic, specific, measurable and
progressive. Initiates interdisciplinary communications and
incorporates recommendations. Consults with a Registered Nurse to
formulate care plans that reflect concerns for holistic patient
needs for the patient, family, and significant others. Assess and
evaluates patients’ response and outcome and makes appropriate
modifications on the care plan. Reviews and updates the care plan
as needed with the assistance of the Registered Nurse to reflect
the patients’ status throughout hospitalization. C. Implements and
monitors appropriate nursing care to meet patients’ needs, and
identifies expected outcomes for planned nursing interventions, and
insures vital information is passed between shifts. Administers
medications according to policy including IV’s and IVPB’s as
prescribed by the primary care provider. Knows actions,
contraindications, precautions, adverse reactions, dosage and route
of administration on all drugs before administering to patient, and
verifies against the medication administration record, (MAR), for
accuracy on each shift and on an ongoing basis. Assists the RN in
monitoring all blood transfusions, after the transfusions has been
initiated by the RN. Notifies the RN of all IV push medication to
be administered. Gives report to other health care personnel
providing accurate and complete information on each patient at the
change of shift, when transferring patients to another unit, and at
intervals throughout the shift. Assesses patients at the beginning
of the shift for acuity of patient needs/problems and based on that
assessment, establishes priority in which work activities are
consistently completed by the end of the shift. Performs basic
nursing skills competently to include but not not limited to:
intake and output, vital signs, collection of specimens/cultures,
dressing changes, personal hygiene for patient, skin care, glucose
monitoring, airway/suctioning, oxygen administration, tube
feedings, urinary catheterization, drains, enemas, and IV
catheterization. Notifies supervisor and physicians of emergency
situations promptly. Recognizes variances in vital signs and
assessments that need to be reported to the RN, supervisor, and the
physician in time appropriate to the variance. Functions
proficiently in unit nursing modalities. Consistently checks for
new Dr.’s orders throughout the shift and implements them in an
appropriate time frame. D. Participates in the health care
educational needs for the client, family and others with regard to
patient care needs, disease prevention, and health maintenance
measures. Assists the RN in developing and implementing an
individualized teaching and discharge plan at time of admission and
throughout the patients hospitalization. Initiates and documents
progressive patient, family, or significant others’ teaching on
admission and throughout hospitalization. Act as a liaison between
patient, physician, family, significant other, and other members of
the health care team. E. Maintains accurate and timely records
according to hospital policy and procedures. Accurately documents
on each shift the nursing patient care activities and the patient
response to nursing interventions and delegated medical management.
Assists with maintaining a patient care plan that is current and
reflects individual needs and preferences of patients and others.
Documents teaching completed and patient/family response in the
patient record. Transcribes and/or co-signs physician orders
completely and accurately according to hospital policy and
procedures. OTHER SIGNIFICANT REQUIREMENTS: Rating A. Maintains
confidentiality while working with confidential matters on a daily
basis. B. Provides service direct to patients while maintaining a
positive patient relations atmosphere. C. Follows aseptic
procedures and precautions when involved in direct patient contact
or handling potentially infectious materials. D. Follows the safety
and infection control guidelines and policies of the hospital. E.
Provides service direct to patients while providing for the care,
Comfort and/or convenience of the patient. F. Maintains a friendly,
collegial and positive attitude and demeanor in all dealings with
patients, families, visitors and/or organization staff. G.
Projects, at all times, a “customer first” attitude and philosophy
in executing assigned responsibilities. H. Interacts with patients,
family members, and other members of the Health care team on a
continuous basis. Requirements: JOB QUALIFICATIONS: A. Education:
Graduate of an accredited School of Vocational Nursing. B. Personal
Job-Related Skills: Effective oral and written communication
skills. C. Licensure, Registry or Certification: Licensed
Vocational Nurse currently Licensed by the State of Texas. Have a
current CPR card and ACLS preferred. D. Experience: Prior Work
Experience: Prefer at least one-year of LVN experience. Technical
Training: Current CPR card. IV therapy course preferred. E.
Physical and Mental Requirements: Duties of the position require
extended periods of standing and walking, bending, stooping,
twisting, reaching, and lifting up to 50 pounds. Occasional heavy
lifting when moving or supporting patients who may suddenly become
weak, helpless, or combative.
