What You’ll Do: Provide skilled in-home nursing visits based on the plan of care Perform vital signs, medication administration, wound care, and catheter care Observe and report changes in patient condition to the RN Case Manager Educate patients and caregivers on medications, safety, and disease management Complete accurate and timely documentation Communicate effectively with the care team What We’re Looking For: Active Texas LVN license (required) Home health experience preferred Strong clinical and communication skills Ability to follow care plans and adapt to patient needs Reliable transportation & valid driver’s license Kinnser/WellSky EMR experience (preferred) Pay & Perks: $50 per visit Flexible, self-managed schedule Independence in the field Meaningful, patient-focused work Consistent patient volume available Work Location: On the road – Galveston, TX and surrounding areas
